Official Inspection Stations - Proof of Insurance Form

The State of Hawaii - Department of Transportation requires proof that an applicant has, in effect, a liability insurance policy issued by an insurance company authorized to do business in the State (Hawaii Revised Statutes, Section 286-27). The acceptable form is the "Official Inspection Station Proof of Insurance Form".
